Nomination Forms for Membership

To nominate someone for membership in the UNI chapter of Sigma Xi:

• Download one of the following versions of the nomination form
• Word
• PDF
• Print it out
• Have the nominee fill in the appropriate parts and attach supporting documents (vita or resume)
• Fill out and sign the nominee portion and have another member second the nomination.
• Send all documentation and dues to

• Decision of the nominations committee will follow soon after
New members will be initiated at the Spring banquet. Cost of membership is $57 for new Full members, $25 for new Student members (associate). There is also a one time initiation fee of $10. For members seeking promotion to Full membership status, a $10 initiation fee is required and if dues are not current, then the appropriate cost for dues is also required.
